Estate Planning Questionnaire - Please review and fill out this form if you are looking to create a Will, Enduring Power of Attorney, and/or a Personal Directive. When these three documents are completed you can appoint someone to make medical decisions on your behalf if you are unable, someone to make financial decisions on your behalf if you lose mental capacity, and to lay out how you would like your estate to be handled by an appointed Personal Representative.PDF

It is helpful that a lot of this information to be included in your Will as this will make the distribution of your Estate easier for your Personal Representative.

Enduring Power of Attorney Questionnaire - Please review and fill out this form if you wish to obtain an Enduring Power of Attorney. An Enduring Power of Attorney will give your designated Attorney the right to make decisions on your behalf respecting your property and financial affairs, should you lose mental capacity or are unable to make these decisions on your own.PDF

A Personal Directive will give your designated Agent the right to make decisions on your behalf regarding your medical care or health decisions, should you lose mental capacity or are unable to make these decisions on your own.PDF
Estate Administration Questionnaire - Please review and fill out this form if you are in the process of administrating an Estate. The information collected will be required by your estate lawyer and/or your Personal Representative when applying for a Grant of Probate or a Grant of Administration. You must have a Grant of Probate or a Grant of Administration in order to distribute the property of an Estate.PDF
